  • Application letter to the Registrar Uganda Nurses and Midwives Council.
  • Brief Curriculum Vitae (C.V).
  • Current employment.
  • Location and the name of the unit.
  • Photocopies of the professional certificates obtained.
  • Professional training i.e institution, year & course undertaken.
  • Recent renewal receipt and license.
  • Recommendation letter from the employers.
