  • <ul><li>Attach certificate of registration where applicable.</li><li>Attach qualification&nbsp; documents for the radiation safety officer, qualified expert and other radiation workers.</li><li>Valid license for possession and use of the source.</li><li>Attach source calibration source certificate from the manufacturer (for radioactive materials).</li><li>Copy of the letter of repatriation from the manufacturer (for radioactive materials).</li><li>Attach a layout of the storage facility at the point of receipt.</li><li>Attach an emergency plan.<br></li></ul>
