Requirements

  • Attach a copy of the license to be renewed.
  • Attach a certificate  of registration  where applicable.
  • Attach a copy of the formal appointment  of the radiation safety officer.
  • Attach qualification certificate of the radiation safety officer.
  • Attach qualification documents for the qualified expert or certificate of approval from Atomic Energy Council.
  • Attach a list of all radiation workers and their corresponding  qualification documents.
  • Attach source calibration certificate from the manufacturer (for radioactive materials)
  • Attach a copy of the letter of repatriation from the manufacturer (for radioactive materials).
  • Attach the layout and design of the facility.
  • Attach the standard certificate (ISO or IEC) if available.
  • Attach a qualified expert's report for category 1 & 2 or a safety assessment report for category 3 & 4.
  • Attach a security and safety plan.
  • Attach a waste management plan for radioactive sources.
  • Attach an emergency plan.
  • Attach a quality assurance programme.
