Requirements

  • Attach copies of the radiation safety officer's qualification certificates and his/ her appointment letter.
  • Attach copies of qualified expert qualification certificates.
  • Valid license from the receiving country.
  • Copy of the source calibration certificate from the manufacturer (for radioactive material).
  • Copy of the letter of repatriation of the source (for radioactive material).
