  • Doctor's commitment letter.
  • Doctor's Annual Practicing Letter.
  • Number of clinics under Doctor's supervision.
  • Registration fees.
Renewal of health unit license
  • Application fees.
  • Doctor not supervising another Clinic.
  • Doctor's commitment letter.
  • Duly filled application form endorsed by local authority.
  • Experience of 3 years.
  • Valid Annual Practicing License.
  • Previous copy of an operating license.
