Obtain a form from the Registrar or download from the website.
Original and photocopies of each of the following documents;
registration certificate, annual practicing license, diploma
certificate/transcript, payment of registration fees.
Professional can only run one health unit.
Professional must have 4 years and above working experience.
Professional must have registered with the council.
Professional opens health unit under his/her area of specialization
Professional should have a valid annual practicing license.
Renewal fees.
Requirements for renewal of practicing license for a health unit:
The form should be signed and stamped.
The form should have a valid signature and stamp from both the inspecting officer and District Health Officer.
