Protect the public and environment from dangers of radiation

Attach a certificate of registration where applicable.
Attach copies of qualification documents of the radiation safety officer and other radiation workers.
Attach formal appointment of the radiation safety officer.
Attach qualification documents of the qualified expert.
Attach source calibration certificate from the manufacturer.
Attach a copy of the letter of repatriation from the manufacturer (for radioactive materials).
Attach a copy of the architectural drawings of the installation and premises.
Attach a copy of the qualified expert's report.
Attach final decommissioning plan.
Attach final radiation survey report.
Attach a quality assurance programme and supporting documents.
Attach procedures for dealing with and reporting abnormal events, incidents and emergencies.
Attach a work breakdown structure and implementation programme.
Attach administrative control procedures for individual tasks.
Attach procedures for the collection and maintenance of records during and after completion of decommissioning.
Any other information that may be required by Atomic Energy Council.
