Requirements

  • Attach a certificate of registration where applicable.
  • Attach copies of qualification  documents of the radiation safety officer and other radiation workers.
  • Attach formal appointment  of the radiation safety officer.
  • Attach qualification documents of the qualified expert.
  • Attach source calibration certificate from the manufacturer.
  • Attach a copy of the letter of repatriation from the manufacturer (for radioactive materials).
  • Attach a copy of the architectural drawings of the installation and premises.
  • Attach a copy of the qualified expert's report.
  • Attach final decommissioning  plan.
  • Attach final radiation survey report.
  • Attach a quality assurance programme and supporting documents.
  • Attach procedures for dealing with and reporting abnormal events, incidents and emergencies.
  • Attach a work breakdown structure and implementation programme.
  • Attach administrative control procedures for individual tasks.
  • Attach procedures for the collection and maintenance of records during and after completion of decommissioning.
  • Any other information that may be required by Atomic Energy Council.
